I approve of my child / children’s involvement in Sew & Co and Chip & Co’s after school programs, hereafter referred to as “the programs”, and have read all conditions on this form.
I give permission for my child/ children to participate in all activities in the programs, including the use of all tools and equipment such as sewing needles, pins, scissors, sewing machines, nails, hammers, screwdrivers, pliers, small vices etc. This also includes non-electric hand drills and non-electric hand saws.
I authorise staff, in the event of accident or illness, to obtain all necessary medical assistance and treatment for my child and agree to meet any expenses attached to such treatment.
I acknowledge that my child will not attend the programs if suffering from an infectious or communicable disease as identified by the Department of Health.
Sew & Co/ Chip & Co does not accept any liability for personal injury sustained by any participant as a result of his or her participation in the programs.
All program fees must be paid in full before the beginning of each term.
I agree that there is one makeup session available per child during the enrolled term if a child misses a class (to be requested and organised by the parent/ guardian).
Unless otherwise notified the programs will continue to operate on public holidays. If a class is missed a student’s one make up session from Point 7 can be organised and requested by the parent/ guardian.
Parents or guardians take full responsibility in picking up students from the programs or making alternative arrangements at the end of each lesson. Sew & Co/ Chip & Co does not take responsibility in monitoring these arrangements.
Sew & Co/ Chip & Co reserves the right to amend Terms and Conditions.
